The first step on a new journey
Whether you are struggling with a safe place to live, education, employment or have other concerns, we are here to help. Our programs are aimed at helping youth, young adults and families build the skills needed to become self-sufficient, independent and productive.

OHB’s Residential Living Program (Group Home A) provides a safe, stable environment for high-school-age boys where they live, learn and grow to become productive young adults. They have the opportunity to attend our OHB School, engage in therapy, and learn workforce skills through our Skilled Trades Program.
Requirements
- Youth must have court involvement and be court-ordered to attend OHB
- Youth must be referred by Nebraska juvenile probation or DHHS
- Youth must be in grades 9-12 and under the age of 19
- Additional documentation may be requested
Referral Process
- The Residential Living Team will review all collateral information provided and schedule an interview and campus tour with the legal guardian or placing agency when applicable.
- If a youth is accepted into the program, additional documentation is required prior to intake. This will be followed by developing a treatment, step-down, and/or reunification plan for the youth and guardians.

OHB’s Crisis Stabilization Program provides a safe, educational, and therapeutic environment for high-school-age boys who have been removed from their home due to a crisis situation. The program gives youth the opportunity to stabilize in a supportive, age-appropriate environment so they can transition home or to a lower level of care.
Requirements
- Youth must have court involvement and be court-ordered to attend Crisis Stabilization. This program does not accept private placements.
- Youth must be referred by Nebraska juvenile probation
- Youth must be in grades 9-12 and under the age of 19
- Additional documentation may be requested

Branching Out is our Independent Living Program for young adults ages 16 to 26 and focuses on helping clients build skills in three core focus areas: education, employment and independent living.
Requirements
- Any young adult who lacks the skills needed to live successfully independently and is eager to actively take steps to achieve education, employment and independent living goals may apply to the Branching Out Program.
- Applicants must be between the ages of 16 to 26 at the time of application.

- Interested youth must first complete an application
- Staff from Branching Out will contact applicants

Jacobs’ Place is a transitional living program that assists young adults ages 17 to 20 who wish to learn the skills needed to live independently. Jacobs’ Place accepts private referrals and youth who have or will “age out” of foster care.

- The Program Manager will interview the applicant.
- A Mental Status Exam may be required.
- If the applicant is accepted into the program, additional documentation may be required prior to intake. An intake meeting is scheduled with the applicant (and legal guardian for applicants under the age of 19)

The Supportive Community Living Program is an additional service offered within OHB’s Branching Out Independent Living Program. It’s for young adults ages 18 to 26 who are seeking safe, affordable living space while working on the skills needed to live independently.
Requirements
- Young adults ages 18 to 26
- Those with children are welcome to apply
- Minimum 90 days of employment
- Program participants must attend bi-monthly meetings with their Independent Living Specialist to ensure ongoing progress towards meeting goals

The Clinical Services & Counseling Program provides a range of behavioral and mental health services designed to support individuals in achieving personal and therapeutic goals. Services include diagnostic evaluations, individual counseling, and outpatient treatment programs.
Requirements
- Individuals seeking services must have active insurance, the ability to make timely personal payments at the Medicaid rate, or a referral from a provider willing to cover the cost of services.
- Upon acceptance into the program, service recipients must verify that their insurance is in-network and provide a current insurance card at their first appointment.
- Services are available to individuals ages 14 and older.
- Service recipients must not be physically aggressive or pose a threat to themselves or others.
- Admission is based on program availability.
Referral Process
- As part of the application process individuals complete an initial diagnostic interview and any additional assessments needed to help determine the most appropriate level of care.

OHB’s Family Support Program helps individuals and families strengthen healthy functioning at home and in the community. The program focuses on building skills that support effective goal-setting and achievement, empowering individuals and families to create positive, sustainable life changes.

- Family Support cannot accept private placements; youth and families must be referred to the program by Probation or DHHS
- Person(s) cannot be physically aggressive or a threat to self or others

OHB’s Reporting Program provides daily activities for teens who are struggling in their current environment and are in need of additional structure and activity. Youth have the opportunity to engage in a number of learning experiences to help them build critical life skills in a safe and supportive environment.
Requirements
- Reporting cannot accept private placements; youth must be referred to the program
- Youth must be ages 13 to 18
- Youth cannot be a fire starter, physically aggressive, or a threat to self or others
